2016 SHS Marching Band & Orchestra Disney Trip Information!
Preparation is well underway for the 2016 Disney Trip! The following information has been made public as we begin developing the list of students and chaperones who will be coming with us on this tremendous trip! Trip payment options & deadlines.
Payments can be made through our online Store (preferred method) or checks/money orders (payable to “SBOB”) can be mailed to Lonnie Stover, SHS Supervisor of Music, 7400 Cornell Rd., Cincinnati, OH 45242
Full Payment Option, $660 due by May 2nd, 2016
3 Payment Option, Payment 1 $250 due by May 2nd 2016, Payment 2 $200 due by August 15th 2016, Payment 3 $210 due by September 12th 2016

Summit Tour & Travel Trip Application & Health Form
This form is required for all students & chaperones going on the trip. This is a 2 sided form and must be submitted in that format. Please print as a 1 page – 2 sided document, complete and return to your child’s director.
Medication Orders from Physician Form
This form is mandatory for any student requiring medication (this includes “over the counter” medication) while on the trip. If your child already takes medication while at school, you do not need to complete this form. The SHS Nurse will provide a copy of this form in this case.
SBOB Financial Aid Information
The Sycamore Band & Orchestra Boosters provide alternative payment plans and, where necessary, financial aid to students with demonstrated need. These options are available for fees related to student participation & trips.
